SERVICE DESCRIPTION:Offers in-home visits by a nurse and assists by conneting parents with community resources to support the family and children's health and wellbeing.
COVID-19 INFORMATION:During COVID your Family Connects visit may be done by phone or by zoom
INTAKE PROCEDURE:Call for information and to request a visit if you have not had one set up at the hospital
FEES:None
ELIGIBILITY:Parents that have given birth within the last 3 months, adopted an infant under 3 months or are fostering are an infant under 3 months are eligible.
SERVICE HOURS:Monday-Friday 8am-5pm
